No. It is a single payment of $269. Your card is valid for 12 months from the payment date and simply expires after that. Nothing renews automatically. If you want a new card, you buy again. The payment is non-refundable once completed.
The Smile Card is individual: one card per person, in the member's name. For a family, add each member at checkout for $200 and every one of them gets their own card.
The card is designed mainly for patients without insurance who want predictable pricing. If you have a PPO plan (we accept Cigna, Delta Dental, UnitedHealthcare, Aetna and MetLife), talk to our team first. Benefits cannot be combined with insurance for the same procedure.
